What is the Mauritius All-in-One Travel Form?

The Mauritius All-in-One Travel Form is an online declaration that brings together travel, personal, accommodation, emergency-contact and health information for passengers travelling to Mauritius. The official digital form generates a confirmation with a QR code, which travellers may need to present before boarding and at the relevant arrival counters.

Current airport guidance states that the form must be completed for all passengers, including minors. It is separate from any visa or entry permission that may apply to your nationality and journey. For passport-specific visa information, see the dedicated guide on whether you need a visa for Mauritius.

The All-in-One Travel Form is an arrival declaration, not a visa. Completing it does not replace any passport, visa, residence permit or other travel document that may be required for your trip.

When should you complete the Mauritius travel form online?

Complete the form before travelling to Mauritius and before airport formalities where possible. The current official guidance requires passengers travelling by air to complete and present the prescribed All-in-One Health Declaration Form before boarding.

The authorities do not set out a general online submission window on the form itself. For that reason, avoid relying on an unofficial deadline: complete it once your flight, accommodation and contact details are settled, then recheck the information against your passport and itinerary before submission.

What you should prepare before starting

Having the relevant details ready makes the Mauritius arrival form easier to complete accurately. The official form asks for information including:

  • Travel details: your mode of travel, country of residence, airline and flight number, date of arrival, travel purpose, intended length of stay, initial port of embarkation and countries visited during the previous six months.
  • Personal details: your name, date of birth, nationality, passport number, occupation, mobile number, email address and permanent address.
  • Accommodation details: the address where you will stay in Mauritius, plus a contact telephone number or email address.
  • Emergency contact: the name and telephone number or email address of a person who can be contacted during your stay.
  • Health declaration: answers about listed symptoms, including fever, sore throat, skin lesions, cough, joint pain and breathing difficulties.

Use the passport or recognised travel document you will actually carry. A mismatch between the form and your travel document can create avoidable questions at check-in or on arrival.

How to fill out the Mauritius All-in-One Travel Form online

1. Start on the official government platform

Use the official Mauritius digital form portal. The government does not charge a processing fee for submitting the official declaration directly. Take care with lookalike websites, especially where a site presents itself as official or suggests that payment can speed up a government decision.

2. Enter your journey information

Select the relevant travel mode and provide your transport and arrival details. Check the flight number, date of arrival and initial port of embarkation against your booking confirmation. If your flight is not shown, use the option provided by the official form rather than choosing a similar flight.

3. Add your identity and contact details

Enter your personal information exactly as it appears in your passport. This is particularly important for names, date of birth, nationality and passport number. Include contact details you can access while travelling.

4. Provide your Mauritius stay and emergency-contact details

Enter the address of your first or planned accommodation in Mauritius. The form also requests contact information for your accommodation and for an emergency contact. Keep your hotel confirmation or host’s details available in case you need to check an address or telephone number.

5. Answer the health questions truthfully

Review each symptom question and answer it accurately. The declaration confirms that the information supplied is correct and complete. Do not guess or copy a previous traveller’s answers, as health circumstances and itineraries are personal.

6. Review, submit and save the QR-code confirmation

Before submitting, review every section on the final declaration screen. After submission, save the PDF confirmation containing the QR code. Current airport guidance allows the QR code to be shown on a mobile device or in printed form; carrying a printed copy as a backup can still be useful if your phone battery, screen or connection is a problem.

Common mistakes to avoid

  • Using a different passport number: enter the document number for the passport or travel document you will present during your journey.
  • Typing an incorrect flight number or arrival date: check both against your latest airline itinerary, particularly after a schedule change.
  • Leaving accommodation details incomplete: prepare the full address and an available contact method before starting.
  • Forgetting a child’s declaration: minors are included in the current requirement, so make sure each travelling child is covered by the required form process.
  • Submitting without checking the final screen: review the declaration carefully before confirming it, rather than assuming it can always be amended later.
  • Confusing the form with a visa: the Mauritius All-in-One Form does not determine whether you need a visa or another entry document.

Is there a PDF version of the Mauritius arrival form?

The official online process produces a PDF document with a QR code after submission. This PDF is your completed-form confirmation, not a blank paper version to fill in instead of the online declaration.

Save the PDF somewhere you can reach without searching through email at the airport. A screenshot of the QR code may be helpful as an additional backup, but retaining the full PDF and a printed copy gives you more than one way to show your confirmation.

Health-related travel checks that may affect your journey

The All-in-One Travel Form includes a health declaration, but it is not the only health-related consideration for every itinerary. Mauritius currently applies additional public-health measures to certain travellers who have departed from, travelled to or transited through the Democratic Republic of the Congo, Uganda or South Sudan during the preceding 21 days.

Because these measures can affect whether a passenger may board, check the latest requirements with your airline and the relevant Mauritian authorities before departure if this part of your recent itinerary applies to you.

Need help completing your Mauritius Travel Form?

You may complete the official declaration yourself through the government portal, without a government processing fee. If you would prefer optional private assistance, choose a service that clearly explains what it does, lets you review the information entered and does not claim to be a Mauritian authority or promise an immigration outcome.

Practical example: completing the form for a family trip

A family travelling from the United Kingdom via Dubai should use the actual passport details for each traveller, the final flight into Mauritius and the full address of their planned accommodation. They should also list the countries visited during the previous six months accurately, including relevant transit or travel history where required by the form.

For children, do not assume that an adult’s confirmation automatically replaces the child’s required declaration. Check that the form process has been completed for every passenger and that each QR-code confirmation is available before leaving for the airport.

If your plans change after completing the form

If your flight, accommodation or passport details change, do not assume that the existing confirmation remains accurate. Return to the official process to check whether you can update the details or whether a new declaration is needed. If you cannot confirm this before travel, seek guidance from the airline or the relevant Mauritian authority.

Keep the completed confirmation with your passport and other travel documents. The form is designed to support arrival formalities; it does not guarantee boarding, entry or admission to Mauritius.
